How to request Waiver of Premium due to

It’s always a good idea to first check that your policy covers the claim. At FWD, you can do this by calling our Customer Connect team on+632 8888 8388, via email or visiting one of our business hubs. Alternatively, you can also check via our customer app Tapp, or the Customer Connect Portal

We also recommend you to submit your claim within 90 days of the event.

Please note that if you're claiming for a waiver of premium, you need to first complete and submit a loss of life claim.

For Waiver of Premium on Owner (WOPO) claim:

  • Original Philippines Statistics Authority (PSA) copy of the deceased policy owner's death certificate
  • Valid government ID of the owner. Must include date of birth, photo, and signature

For Waiver of Premium on Insured (WOPI) claim:

  • Valid government ID of the owner. Must include date of birth, photo, and signature

If death was due to accident or injury:

  • Original police report or police traffic incident report, if any (certified)
  • Copy of driver's license if accident happened while the insured person was driving

What does ‘certified’ mean?

This means the document must be checked and signed by the relevant authority – for example, the police or a hospital.
