How to file a claim

Main requirements:

  1. Original copy of the Certified True Copy of the Death Certificate of Insured issued by the Local Civil Registrar (LCR) or Original copy issued by the Philippines Statistics Authority (PSA)
  2. Completely accomplished, dated and signed Claimant’s Statement forms of the beneficiaries
  3. Proof of relationship:

For spouse beneficiary: Original copy of Marriage Certificate issued by the Philippines Statistics Authority (PSA) For child beneficiary: Original copy of Birth Certificate issued by the Philippines Statistics Authority (PSA)

For Daughter with Single status in the contract and Married at time of claim: Original copy of Marriage Certificate issued by the Philippines Statistics Authority (PSA) or both copies of IDs with Single Status and Married Status

  1. Copy of Insured’s government ID with date of birth, photo and signature
  2. Copy of Beneficiary/ies government ID with date of birth, photo and signature.
  3. Proof of Bank Account (Please submit a copy of any of the following. Make sure your full name and account number are visible:
      1. Statement of account
      2. ATM card
      3. Validated deposit slip
      4. Passbook
      5. Screenshot of your online bank account

If the policy is contestable:

  1. Completely accomplished, dated and signed Attending Physician’s Statement form
  2. Certified True Copy of Complete medical/hospital records of insured from previous and recent consultations and confinements including but not limited to Medical Certificate, Admitting History Record, Clinical Abstract, Discharge Summary, lab result and all diagnostic results.

If there is a discrepancy on the Name or Date of Birth of Insured in the Insurance Application vs the submitted Death Certificate and copy of ID:

  1. Original copy of Birth Certificate of the Insured issued by the Philippines Statistics Authority (PSA)

If death is due to violence or accident:

  1. Original or Certified True Copy of Police Report
  2. Original or Certified True Copy Autopsy Report / Post-Mortem Examination Report
  3. Copy of Driver’s License if the accident happened while the insured was driving a vehicle
  4. Notarized Sworn Statement of Witness/es with attach copy of the witness/es valid ID with photo and signature
  5. Other evidence of Accident, if any: Newspaper, Picture, etc

If the beneficiary is a minor and share of proceed is less than P500,000.00:

  1. Notarized Affidavit of Legal Guardianship
  2. Copy of government ID with photo and signature of the Legal Guardian

If the beneficiary is a minor and share of proceed is greater than P500,000.00:

  1. Petition for Guardianship
  2. Letters of Guardianship
  3. Oath of Office of Guardianship
  4. Guardian’s Bond approved by Court
  5. Court Decision
  6. Copy of government ID with photo and signature of the Legal Guardian

If beneficiary is Estate:

  1. Deed of Extra-Judicial Settlement duly registered in the Registry of Deeds near the place of Insured’s residence
  2. Affidavit of Publication
  3. Newspaper clippings where the EJS was published
  4. Proof of relationship of the heirs to the deceased
  5. Affidavit of Self-Adjudication if the deceased has only one heir
  6. Copy of government ID with photo and signature of the Estate

If policy was assigned to Persons:

  1. Certificate of Indebtedness

If policy was assigned to Corporations:

  1. SEC Registration, Articles of Incorporation, By-Laws
  2. Board Resolution and /or Secretary’s Certificate
  3. Certificate of Indebtedness
  4. Copy of government ID with photo and signature of the authorized signatory/ies and the corporate secretary

If death happens outside the Philippines:

  1. Death Certified by Coroner overseas
  2. Any of the following documents to support validation of death abroad:
    • Certificate of Cremation
    • Burial Permit
    • Airway Bill
    • Bureau of Quarantine Certificate
    • Marine Protest Report
    • Ship’s Passenger manifest
    • Obituary

** All documents related to and coming abroad should be duly authenticated by the Philippine Embassy or Consulate Office nearest the residence of claimant

** All documents must be translated in English"

If beneficiary is a U.S. Person or has U.S. Indicia

  1. W-9 or W-8BEN form may be downloaded from the IRS website at www.irs.gov/forms-pubs

If claim is made through a Representative

  1. Special Power of Attorney (SPA)
  2. One (1) valid ID bearing photo and signature
