I have the claim forms and documents ready. When should I file my claim?
We request that all claims (including loss of life) are made within 90 days. One of our dedicated claims ambassadors will then get in touch with you to confirm that we’ve received your claim and advise on next steps.
In case you’re not able to submit within the 90-day period, don’t worry! We will not invalidate nor reduce the claim if we find that it’s not practical to submit the requirements within the allowable time.
I’m currently overseas. Can I still make a claim?
Yes, no problem.
However all documents coming from abroad will need the Philippine Consular Office’s authentication. You’ll need to bring your claim documents to the Philippine Consular Office to get them authenticated. This may involve making an appointment so do check in advance before turning up. A simple online search will tell you where your nearest Philippine Consular Office is. If not, feel free to get in touch with our team and we’ll find out on your behalf.

Can somebody else file a claim on my behalf?
Yes. Please provide a Special Power of Attorney (SPA) document with your claim. This needs to include valid IDs for both of you.

Can I increase my life coverage?
No problem. To do this, simply fill out and submit the following two forms:
Policy change form - dated, signed by policyholder and witness (can be your advisor or an FWD branch executive)
Health statement form
We’ll then send you an updated sales illustration and will let you know the premium payment difference. Please have a read of all the terms, then sign and date it and send it back to firstname.lastname@example.org.
You can only increase your coverage within one year from your policy's start date.
You will need your assignees or irrevocable beneficiaries’ consent to reduce your coverage.
I want to change my life insurance beneficiary. How do I do that?
Please head along in-person to any FWD business hub and submit two copies of the Policy change form - dated, signed by policyholder and witness (can be your advisor or an FWD branch executive). This is because we need to authenticate all the information.
If your current beneficiary is irrevocable, please have them sign the form.
If you’re designating your spouse as a new beneficiary, please provide a marriage contract.
If your intended beneficiary is a minor, please name a trustee on the form, along with a notarized deed of trust form.
