No problem. To do this, simply fill out and submit the following two forms:
Policy change form - dated, signed by policyholder and witness (can be your advisor or an FWD branch executive)
Health statement form
We’ll then send you an updated sales illustration and will let you know the premium payment difference. Please have a read of all the terms, then sign and date it and send it back to posconnect.ph@fwd.com.
Please remember:
You can only increase your coverage within one year from your policy's start date.
You will need your assignees or irrevocable beneficiaries’ consent to reduce your coverage.
Please head along in-person to any FWD business hub and submit two copies of the Policy change form - dated, signed by policyholder and witness (can be your advisor or an FWD branch executive). This is because we need to authenticate all the information.
If your current beneficiary is irrevocable, please have them sign the form.
If you’re designating your spouse as a new beneficiary, please provide a marriage contract.
If your intended beneficiary is a minor, please name a trustee on the form, along with a notarized deed of trust form.
That’s fine. Although bear in mind that you can access FWD Tapp your policy anytime, anywhere via or our Customer Connect Portal. However, if you’d like to receive a physical copy, we can send you a duplicate. Please first send us the following:
