  • Go to the FWD Life branch nearest you. You may also give the documents to your Financial Wealth Planner or Financial Solutions Consultant

Submission Timeline

  1. Please email soft copies of the documents as a notice for filing a claim in advance or within thirty (30) days from the date of loss.
  2. Please submit original, completed claim forms along with their required supporting documents within ninety (90) days after the time proof is required.

Get status confirmation

  • We will send updates via email upon receiving of the complete documents. A status confirmation will also be given if the claim has been approved or if additional requirements are needed.  If you do not hear from us, you may email CorpCareClaims.ph@fwd.com

Receive benefits

  • A final update will be sent via email that the benefit payout is ready for release.  Payouts will be done via check.
