FWD Gamer Survey Promo Mechanics

Promo period is from September 22 to Oct. 29, 2023.
This promo is open to all online or live audience of Mobile Legends Bang Bang: Philippine League Season 12 who successfully completes the FWD gamer survey.
The survey will be open during the live matches on the following weekends:

A participant is allowed to answer the survey only once and shall be considered as a raffle entry. Multiple entries are not allowed.
There will be 1,000 winners of Mobile Legends in-game items and they are also qualified for the grand raffle. Prizes are as follows:

1. 1st Prize: Samsung Gaming LCD Monitor
2. 2nd Prize: Sharkoon Skiller Gaming Chair
3. 3rd Prize: Logitech Gaming Headset
4. Consolation Prizes: 7 winners of PHP 1,000 GCash

Participants who successfully complete the survey will receive their Mobile Legends in-game items within 7-15 days after successful completion of the survey.
Winners of the grand raffle will be determined through an electronic raffle draw scheduled on November 15, 2023, at the 19/F W Fifth Avenue Building, 5th Avenue corner 32nd Street, Bonifacio Global City, Taguig City.
Winners of the Mobile Legends in-game items will be notified via email 5-7 business days after they complete the survey. Winners of the grand raffle will be notified via email on November 3, 2023. They will be provided with instructions on how to redeem their rewards.

OTHER TERMS AND CONDITIONS

FWD reserves the right to cancel the promo and/or amend the promo mechanics upon notice.
All issues pertaining to the campaign/promo shall be resolved by FWD and its decision on such matters, including the mechanics, winners, prizes, shall be final.
Any and all expenses related to the claiming and use of the prizes shall be for the sole account of the winner.
FWD reserves the right to choose an alternative winner if the winner cannot be contacted and/or fails to qualify after 60 days of notice to winner

ATTESTATION

1. Consents and allows to the processing by FWD Life Insurance Corporation (“FWD") of his/her personal data in accordance with FWD’s privacy policy at https://www.fwd.com.ph/privacypolicy/ , for marketing and research purposes and consistent with the processes described above;
2. Agrees to hold FWD free and harmless from any loss, injury, or damage caused by or arising from his/her participation in the promotion or caused by or arising from his/her acceptance and/or use of the prize;
3. agrees to cooperate and participate in any advertising or publicity activities of FWD relating to the marketing coverage.
4. If chosen as the winner, the participant agrees that FWD may publicize the same in such mode and manner as FWD considers fit. The winners’ names may be published on the FWD website and social media channels.
