  • If needed for Waiver of Premium on Owner:

    • Certified True copy - Complete Medical and Hospital Records
    • If Insured works as an Employee: Daily Time Record, Certificate of Leave of Absence
    • Any medical requirements (under Section 12 Definition of Covered Critical Illnesses as stated in your contract provision)
  • Additional documents (if needed)

    If disability or death is due to an accident:


    • Certified True copy - Police Report
    • Certified True copy – Police Traffic Incident Report
    • Notarizes Statement of Witness/es
    • Photocopy of Driver’s License (if accident happened while Insured was driving the vehicle)
    • Certified true copy of Owner’s Death Certificate issued by the local civil registrar or issued by the National Statistics Office (NSO).

  • Claimant’s Statement: Waiver of Premium Claim Form

    To be completed by the Insured.

  • Attending Physician’s Statement: Waiver of Premium Claim Form

    To be completed by the attending doctor.

Get status confirmation

We will send updates via SMS and email upon receiving your complete documents. A status confirmation will be given if your claim has been approved or if additional requirements are needed. If you do not hear from us, you may email ClaimsConnect.ph@fwd.com.

Receive benefits


A final update will be sent via SMS and email that your benefit payout is ready for release. Payouts can be done via crediting to the designated beneficiaries’ bank accounts or via check.
