Submit Complete Claim Requirements

  • Hospital Cash Benefit Claim – Claimant Statement Form
    Hospital Cash Benefit Claim – Attending Physician Statement Form

Basic Hospital Cash Benefit Claim Requirements

  1. Duly accomplished and signed Claimant’s Statement Form

  2. Copy of Hospital Records – complete records of confinement including Medical/Clinical Abstract, Admitting History Record, Discharge Summary Record and Hospital Statement of Account.
  3. One (1) Valid government ID of Claimant
  4. For accident related hospitalization, the following are needed;
    • Copy of Police Report or Police Traffic Incident Report, if any.
    • Copy of Driver’s License if accident happened while the Insured is driving.

Note

  1. Additional requirements may be prescribed as necessary to process the claim.

  2. Claims within the contestability period (policy issued within the first 2 years) are subject for a thorough verification.
  3. Our Claims Ambassador will connect with you to update you on the additional requirements needed.

Receive Status Confirmation

Claims Ambassador will connect with you to provide update on your claim such as decision on the claim or request for additional requirements.

  • TURNAROUND TIME: Claims evaluation starts once complete requirements are submitted. The following is our standard processing time for claims:
    • For Contestable Claims (contracts within 2 years): 30 calendar days upon complete submission of claim requirements.
    • For Incontestable Claims (contracts beyond 2 years): 5 working days upon complete submission of claim requirements.
  • Note: Evaluation may take longer for contestable cases. Claims Ambassador will closely coordinate with you for any updates.

Receive Benefits

  • You may receive your benefits through Cheque
  • Directly credited to your personal bank account


A final update will be sent via SMS and email that your benefit payout is ready for release. Payouts can be done via crediting to the designated beneficiaries’ bank accounts or via check.
