  • If confinement is due to sickness or illness:


    • Certified True copy - Complete Medical Record
    • Certified True copy - Admitting Medical History Record and Discharge Summary
    • Certified true copies of any of the following: Hospital Statement of Account (with date/time of admission and discharge), Hospital Certification (with date/time of admission and discharge)
  • If confinement is due to accident or injury:


    • Certified True copy - Complete Medical Record
    • Certified True copy - Admitting Medical History Record and Discharge Summary
    • Certified true copies of any of the following: Hospital Statement of Account (with date/time of admission and discharge), Hospital Certification (with date/time of admission and discharge)
    • Certified True copy - Police Report
    • Certified True copy - Police Traffic Incident Report
    • Notarized Statement of Witness/es
    • Issued narration of incident
    • Photocopy of Driver’s License (if confinement is due to vehicular accident)

  • Claimant’s Statement: Hospital Cash Benefit Claim Form

    To be completed by the Insured.

  • Attending Physician’s Statement: Hospital Cash Benefit Claim Form

    To be completed by the attending doctor.

  • Go to the FWD Life branch nearest you. You may also give the documents to your Financial Wealth Planner or Financial Solutions Consultant

Submission Timeline

  1. Please submit the completed claim form with required supporting documents to us within 90 days after the time the proof is required. We shall provide the necessary forms for filing proof of loss upon receipt of the notice of claim
  2. Please submit original, completed claim forms along with their required supporting documents within ninety (90) days after the time proof is required.

Get status confirmation

We will send updates via SMS and email upon receiving your complete documents. A status confirmation will be given if your claim has been approved or if additional requirements are needed. If you do not hear from us, you may email ClaimsConnect.ph@fwd.com.

Receive benefits


A final update will be sent via SMS and email that your benefit payout is ready for release. Payouts can be done via crediting to the designated beneficiaries’ bank accounts or via check.
